REQUIREMENTS AND PROCEDURES
Please provide the following:
Birth Certificate - Please bring in your student's birth certificate, we will make a copy
Proof of Parent/Guardian Residence: Two separate documents tied to the home are required to verify residence. Reasonable evidence of residency may be established by documentation, including but not limited to any of the following:
Current Utility Bills Car Insurance Bill State Agency Mail Housing Authority Voucher Current W-2 Forms Property Tax
Rental or Lease agreement with parent name, student name, and address, as well as manager or owner’s name and phone number.
Immunization Record: Please provide a copy of your child’s current immunization record. Must include Tdap pertussis.
Previous School Records: Before your child can enroll we must have all of the following:
- Official Transcript
- California HS Exit Exam Scores (11th and 12th grade only)
- Transfer Grades
- Attendance & Discipline Record
- Withdrawal Form
- CELDT Scores (If applicable)
IF YOUR STUDENT IS IN SPECIAL EDUCATION (has a current I.E.P)
